Job Description:

Lincoln Community Health Center, Inc., a federally qualified community Health Center (FQHC) located in Durham, NC is recruiting for an Early Intervention Clinic Behavioral Health Professional. Durham is known as the Bull City, the City of Medicine, the Tastiest Town in the South, and the Startup Capital of the South. It’s a two-hour drive from the beach, a three-hour drive from the Blue Ridge Mountains, and is North Carolina’s fourth largest city by population. Along the East Coast, Durham is located conveniently along I-85. Occupational Summary
Provides integrated substance abuse/mental health treatment to HIV infected patients through assessment, treatment planning, therapeutic interventions, case management, outreach, active coordination of care between community-based providers (case managers) as well as the ID clinic providers. Provides integrated substance abuse treatment to patients of Healthcare for the Homeless. The BHP ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family member’s needs and personal support systems. Actively supports the Lincoln Community Health Center’s mission of service and education.

Knowledge, Skills, and Abilities
A general knowledge of Durham community and its resources is required. Knowledge of HIV/AIDS through seminar and coursework required. Interviewing, facilitation, and computer skills required. Cultural competence through ability to communicate effectively, verbally, and in writing to diverse populations including patients, staff, and general public required. Ability to perform with minimal supervision desired.

Education
MSW, and licensed or provisionally licensed LCSW and LCAS. Some knowledge or previous work experience in the health field especially related to HIV/AIDS care.

Experience
One (1) year of working in a community/public health setting. General knowledge of the Durham community desired.
