NOTICE: Registration is closed and the Conference is Full.
This conference is a professional training opportunity providing high quality sessions applicable to clinicians and clinical support staff in safety net healthcare organizations such as community health centers, health departments, rural health clinics, and school-based health centers. Nationally recognized health care experts share knowledge with safety-net providers on cutting-edge trends and quality improvement methods.

The Clinical Conference on Quality & Chronic Disease is a professional training opportunity providing continuing education at a low cost. Nationally recognized health care experts share knowledge with safety-net providers on cutting-edge trends and quality improvement methods.

 

REGISTRATION

Through Thursday, April 4th ( or until full ) 2019: $200.00
(includes workshop materials, meals, and breaks)

Registration is closed and the Conference is Full.

No onsite registrations.

Online registration is always preferred, and required if paying by credit card; a paper registration form can be downloaded here for mailed check payments only.

The North Community Health Center Association invites you to become a sponsor or exhibitor of the 2019 Clinical Conference on Quality & Chronic Disease. Our target audience includes: community health centers, local health departments, rural health centers, hospitals and other safety net providers across North Carolina. This is your opportunity to directly interface with clinicians and clinical support staff from community health centers, rural health centers, health departments, university health systems and others.

Registration Fees

Registration Fee is $200.00 Registration is Closed.

No onsite registrations. please.

Late Fee will apply.

Refund Policy: Written notice of cancellation must be postmarked (or received via email) by Thursday, March 21, 2019 for a partial refund, less a $50.00 administrative fee. No refunds will be given after March 21, 2019. Participant substitutions are allowed upon request/approval. Please allow sixty days post-conference for refunds to be processed.

Administrative Fees: NCCHCA will charge a $25.00 administrative fee for each registration that has been previously processed and/or requires re-submission due to incorrect selection of registration fees or denial of payment for any reason.

Attire: Business casual attire is recommended during the Clinical Conference. Please dress in layers as temperatures in meeting rooms can vary.
