2008-2009 Patient Safety and Clinical Pharmacy Collaborative

North Carolina has the distinction of having 2 teams (Lincoln Community Health Center and Piedmont Health Services) accepted into this premier pilot that strives to improve patient safety and to fully integrate clinical pharmacy services into primary care and the continuum of care.  Our teams are participating in this national effort alongside a wide variety of healthcare settings, including private practices, hospitals, integrated care delivery systems and more.

In contrast to past Collaboratives conducted by HRSA, team consisted of partners from across the community rather than being individuals from the same organization. This allows various healthcare settings who see the same patient to redesign systems of care, especially those where patient handoffs occur, where there is a greater chance for error to occur.
