The Janet Reaves Memorial Conference on Quality and Chronic Disease will assemble health care leaders who are actively engaged in providing quality health care to the citizens of North Carolina.
This conference is offered in loving memory of Janet Reaves, RN, MPH. Janet's dedication to public health and her tireless advocacy on behalf of those with chronic disease had a significant impact on North Carolinians.

Who Should Attend: Community, rural, public, hospital-based, private or university-based health professionals.
Due to the generosity of our conference sponsors, there are a limited number of scholarships available. These will be awarded on a first-come, first-served basis. You may elect 1-day registration, full registration, and/or the vision screening certification training.
To receive a scholarship, complete & fax this form. In the registration fees area please indicate "SCHOLARSHIP REQUEST". Fax to 919-469-1263, Attention: Marti Wolf. You MUST print and fax the Registration Form if you are requesting a scholarship.
CEUs will be available for the conference: 6.25 contact hours for both days without the vision training; 8.75 contact hours with the vision screening certification training.
Lunch (Sponsored by GlaxoSmithKline Vaccines) and dinner (Sponsored by LabCorp) will be provided on Thursday, Feb 18 . A full breakfast and lunch will be provided on Friday, Feb 19.
DEADLINE: Your scholarship request must be received by 5 pm on Wednesday, February 3, 2010. You will be notified by 5 pm on Friday, February 5, 2010 as to the status of your request.
